Mind Body Mouth, its all connected

7-13 August 2023

Dental Health Week (DHW) is the Australian Dental Association’s (ADA) major annual oral health campaign. This year, DHW is focusing on the mouth and whole-body connection. There are links between diseases in the mouth and diseases in the body. Bacteria in the mouth can travel to different sites of the body and inflammation in the mouth can increase the body’s overall inflammation. Six conditions have been explored in how they link with the mouth:

  • Heart Disease
  • Inflammatory Bowel Disease
  • Lung Conditions

Periodontal status of women with excessive gestational weight gain and the association with their newborns’ health
Women with high gestational weight gain also showed a high prevalence of hypertension and periodontitis during pregnancy, and persistent periodontitis after delivery. High gestational weight gain was related to high BMI of newborns. International dental journal 5 June 2020

Association between dental caries and adverse pregnancy outcomes
Poor oral health is not only associated with diabetes and cardiovascular disease but adverse pregnancy outcomes. However the influence of dental caries on pregnancy is unknown. The aim of this study was to evaluate the association between dental caries and adverse pregnancy outcomes and the effect of treatment for dental caries on adverse pregnancy outcomes. Adverse pregnancy outcomes and maternal periodontal disease: An overview on meta-analytic and methodological quality

Alzheimer's disease

Oral health of individuals with dementia and Alzheimer's disease: A review
This paper explores the epidemiological evidence about oral health of individuals with neurodegenerative conditions of Alzheimer's disease (AD) and dementia. The explored literature from 22 studies shows that oral health parameters of oral health and levels of oral inflammatory markers were deranged and exaggerated in patients suffering from AD and dementia. Many studies have observed poor oral hygiene as result of lack or irregularity in toothbrushing. Journal of Indian Society of Periodontology 1 March 2021

Poor oral health predicts cognitive impairment in elders
Periodontitis, tooth defects, or arch integrity was associated with poor performance of cognitive domains (memory, attention, and executive function) over time. A oral health evaluation may serve as predictors of the preclinical phase of dementia in its upcoming years. Alzheimer's & dementia 16 June 2023

Association between dementia severity and oral hygiene management issues in older adults with Alzheimer’s disease: A cross-sectional study
Oral hygiene management issues vary across types and clinical stages of dementia. The authors aimed to clarify the issues related to oral hygiene management in older adults with Alzheimer’s disease (AD) according to stages defined by the Functional Assessment Staging of Alzheimer’s Disease (FAST). Association of oral health with Alzheimer's disease: A review
The relationship between oral infections, inflammation and some chronic non-communicable diseases highlights the importance of the mouth to the body and shows how oral health affects systemic health. Specific relationships have been identified with oral infection/inflammation and certain systemic diseases and conditions like cerebrovascular disease, diabetes mellitus, cardiovascular diseases, pregnancy outcomes and respiratory diseases. Another very interesting link is the relationship between oral infections and cognitive impairment, particularly the type of dementia known as Alzheimer's disease. Oral health knowledge and habits of people with Type 1 and Type 2 Diabetes
There is a need for proper oral health education for PwD. Trained diabetes advocates could be core messengers. However, interdisciplinary cooperation is mandatory for both education and the clinical aspect of diabetes care. For example, diabetes nurses need to be educated with the help of dentists or oral hygienists. International dental journal 8 September 2021

Oral health, Diabetes, and inflammation: Effects of oral hygiene behaviour
Findings from this study highlight the importance of improving oral health and oral hygiene practice to mitigate inflammation. Further research is needed to assess the longer-term effects of reducing inflammation. International dental journal 29 November 2021

Association between periodontal disease status and risk of atrial fibrillation: a nationwide population-based cohort study
The author’s findings suggest that changes in periodontal disease status impact the risk of atrial fibrillation. Management of periodontal disease may help prevent atrial fibrillation. BMC oral health 8 July 2023

The link between different infection forms of Porphyromonas gingivalis and acute myocardial infarction: a cross-sectional study
The distant invasion and colonization of Porphyromonas gingivalis (Pg) were the risk factors of acute myocardial infarction (AMI). They also affected the severity of coronary heart disease (CHD), indicating that periodontitis severity and distant invasion of periodontal pathogens were related to CHD. The presence of Pg was likely able to drive systemic inflammation, suggesting that there was an inflammatory relationship between periodontitis and AMI. BMC oral health 2 February 2023

The inflammation link between periodontal disease and coronary atherosclerosis in patients with acute coronary syndromes: case–control study
Patients with acute myocardial infarction have worse periodontal status compared to people without coronary heart disease. Greater severity of periodontitis, plaque accumulation and bleeding on probing are associated with acute myocardial infarction. Periodontitis is a risk factor for myocardial infarction and also affects the degree of post-infarction left ventricular damage, which means that there is an inflammatory link between these two diseases. Inflammatory Bowel Disease

Patients with inflammatory bowel disease have more oral health problems and higher costs of professional dental care than healthy controls: the Periodontitis Prevalence in ulcerative Colitis and Crohn's disease (PPCC) case-control study
IBD patients have more often oral health problems and higher expenses for professional dental care compared to matched controls. This included problems with IBD-related oral lesions, but these are rarely addressed by the medical- or dental team. Journal of periodontology 19 July 2023

The interplay between oral microbiota, gut microbiota and systematic diseases
In this review, the authors discuss possible routes and factors of oral microbiota to affect gut microbiota, and the contribution of this interplay between oral and gut microbiota to systemic diseases. Although most studies are association studies, recently, there have been increasing mechanistic investigations. This review aims to enhance the interest in the connection between oral and gut microbiota, and shows the tangible impact of this connection on human health. Journal of oral microbiology 15 May 2023

Lung conditions

Periodontal status and chronic obstructive pulmonary disease (COPD) exacerbations: a systematic review
The data supports possible association between poor periodontal health, the frequency of exacerbations, hospitalisation and quality of life in COPD patients. The evidence is of moderate to low certainty and is limited by high risk of bias suggesting the need for well-designed and adequately powered randomised controlled trials, to inform future research and clinical practice. Oral health and wellbeing of patients with idiopathic pulmonary fibrosis
The study findings prompt the need for regular dental consultations and careful observation of dental prophylaxis in patients with Idiopathic pulmonary fibrosis (IPF). The differences noted in the quality of life assessed using a GOHAI questionnaire need a further validation in larger cohort studies. Journal of health studies and medicine 31 May 2023

Oral health literacy, knowledge and perceptions in a socially and culturally diverse population: a mixed methods study
This study aims to investigate oral health literacy (OHL) in a socially and culturally diverse population of Australian adults visiting a public dental clinic in Western Sydney. Strategies aimed at redressing disparities in oral health status should include improving access to oral health information. The focus should be on the impact poor oral health has on general health with clear messages about prevention and treatment options in order to empower individuals to better manage their oral health. Periodontal disease and other adverse health outcomes share risk factors, including dietary factors and vitamin D status
For nearly a century, researchers have associated periodontal disease (PD) with risks of other adverse health outcomes such as cardiovascular disease, diabetes mellitus, and respiratory diseases, as well as adverse pregnancy outcomes. Those findings have led to the hypothesis that PD causes those adverse health outcomes either by increasing systemic inflammation or by the action of periodontopathic bacteria. This review describes the evidence that proinflammatory and prohyperinsulinemia diets and low vitamin D status are important risk factors for PD and other adverse health outcomes. Tools evaluating child oral health–related quality of life
This review identified 12 tools developed for evaluating oral health–related quality of life (OHRQoL)  in preschool children, 7 of which were tailored for preschool age. The 12 tools were validated but incomprehensive due to the subjective and multidimensional nature of the OHRQoL concept. Researchers can choose a suitable tool for their studies by understanding the basic characteristics and item setting of the tools. Researchers can have an overview of the tools developed for evaluating OHRQoL in preschool children. They can use the findings from this review to choose a suitable tool for their studies regarding the OHRQoL in preschool children. International dental journal 22 July 2023

A cross-sectional study of perceived dental treatment needs and oral health status in community-dwelling older Australian men: The concord health and ageing in men project
Most older men viewed their oral health positively, and the majority of those with objectively assessed needs, did not perceive themselves as requiring dental treatment. Loss of functional tooth units (FTUs) appears to alert participants to seek treatment, but not the presence of dental caries or periodontitis. International dental journal 27 January 2021

A meta-analysis comparing toothbrush technologies on gingivitis and plaque
Oscillating-rotating toothbrushes offer superior results for transition to health, gingivitis, and plaque reduction compared with manual and sonic brushes. The most advanced oscillating-rotating model offers enhanced efficacy vs traditional models. International dental journal 21 July 2023

Improving patient well-being as a broader perspective in dentistry
Patient well-being encompasses the physical, mental, psychological, and social health of an individual. To adequately treat an individual and increase their quality of life, whole-person, patient-centred care needs to be utilised. This review aims to concisely summarise ways to improve patients’ well-being through and in dentistry. Oral health is tied to one's quality of life through oral function, overall health, self-perception, social acceptance, and social interaction. International dental journal 19 June 2023

Oral health and individuals with a lived experience of an eating disorder: a qualitative study
The need for oral health promotion while experiencing an eating disorder was evident, however, dentists can often be a costly option. Non-dental health professionals like dietitians working with clients with an eating disorder may be an acceptable alternative for closing this gap. Journal of eating disorders 17 July 2023

The association between periodontal diseases and helicobacter pylori: an updated meta-analysis of observational studies
Based on the results of this meta-analysis, the association between Helicobacter pylori infection and the periodontal disease is evident. BMC oral health 26 July 2023

Association between sleep duration and dental caries in a nationally representative U.S. population
Findings of this cross-sectional representative study of the U.S. population revealed a statistically significant negative association between sleep duration and dental caries. In this study, individuals who slept < 7 h/night were more likely to exhibit dental caries. BMC oral health 18 July 2023

Correlates of poor oral health related quality of life in a cohort of people who use methamphetamine in Australia
Overall, VMAX cohort participants reported reduced oral health related quality of life (OHRQoL) levels. Our findings highlight the need for upstream interventions to improve the OHRQoL of people who use methamphetamine, with specific focus on those living in rural locations. Further research on the links between OHRQoL and mental health among people who use methamphetamine is required. BMC oral health 13 July 2023

Peri-implant inflammation in waterpipe users and cigarette smokers: An observational study
Waterpipe usage is not less hazardous to peri-implant tissue health than conventional cigarette smoking. It is imperative to caution patients with dental implants about the detrimental effects of tobacco products on oral health. International dental journal 9 April 2023

An observation on the severity of periodontal disease in past cigarette smokers suffering from Rheumatoid Arthritis: Evidence for a long-term effect of cigarette smoke exposure?
Based on this observation The authors hypothesize that chronic cigarette smoke might bring about permanent changes in the periodontal tissues, leading to their hypersensitivity to inflammatory challenges. Journal of oral health 14 April 2023

Patterns and predictors of periodontal disease and tooth loss among users of smokeless tobacco
The study found statistically significant association between duration in years of using SLT and periodontal disease including gingival recession, tooth mobility and tooth loss. However, no significant results were found between retention during single use and frequency of SLT use per day. However, the link of these factors with the periodontal disease cannot be eliminated.
