World Kidney Day

9 March 2023

World Kidney Day is a global campaign aimed at raising awareness of the importance of our kidneys. This year’s theme is Preparing for the unexpected, supporting the vulnerable! Therefore, preparation for unexpected events is incredibly important for kidney patients.

  • Policymakers need to adopt integrated health strategies that prioritize prevention, early detection, and management of NCDs, including kidney disease.
  • Health care services should provide equitable and proper access to care for chronic patients in times of emergency.
  • Governments should include emergency preparedness plans in the management and detection of NCDs and favor the prevention of these conditions.
  • Patients should plan for emergencies by preparing an emergency kit that includes food, water, medical supplies, and medical records.

Competing risks of death and kidney failure in a cohort of Australian adults with severe chronic kidney disease
For older Australians with incident stage 4 chronic kidney disease, the risk of death is higher than that of kidney failure, and the latter risk declines with age. Clinical guidelines should recognise these competing risks and include recommendations about holistic supportive care, not just on preparation for dialysis or transplantation. MJA 17 January 2022

Burden of kidney failure from atheroembolic disease and association with survival in people receiving dialysis in Australia and New Zealand: a multi-centre registry study
Survival outcomes on dialysis are worse for individuals with KF from atheroembolic disease compared to those with other causes of KF, probably due to patient demographics and higher comorbidity. BMC nephrology 2 December 2021

Chronic kidney disease: summary of updated NICE guidance
What you need to know

  • The updated guideline does not recommend adjusting the estimation of glomerular filtration rate (GFR) in people of African-Caribbean or African family background
  • Screen people at risk of chronic kidney disease (CKD) using estimated GFR (eGFR) and albumin to creatinine ratio (ACR)
  • Use the four-variable Kidney Failure Risk Equation instead of eGFR threshold for referral
  • Refer adults with CKD and a five year risk of needing renal replacement therapy of >5% (measured with the Kidney Failure Risk Equation) for specialist assessment
  • Offer a sodium-glucose cotransporter-2 (SGLT2) inhibitor, in addition to an optimised dose of angiotensin converting enzyme (ACE) inhibitor or angiotensin receptor blocker (ARB), to people with type 2 diabetes, an ACR of ≥30 mg/mmol, and who meet the criteria in the marketing authorisation (including relevant eGFR thresholds)

Survival for waitlisted kidney failure patients receiving transplantation versus remaining on waiting list: systematic review and meta-analysis
Kidney transplantation remains the superior treatment modality for most patients with kidney failure to reduce all cause mortality, but some subgroups may lack a survival benefit. Given the continued scarcity of donor organs, further evidence is needed to better inform decision making for patients with kidney failure. BMJ 1 March 2022

Timing of dialysis initiation to reduce mortality and cardiovascular events in advanced chronic kidney disease: nationwide cohort study
Very early initiation of dialysis was associated with a modest reduction in mortality and cardiovascular events. For most patients, such a reduction may not outweigh the burden of a substantially longer period spent on dialysis. BMJ 29 November 2021

Sex differences in mortality among binational cohort of people with chronic kidney disease: population based data linkage study
Compared with the general population, female patients had greater excess deaths, worse relative survival, and more years of life lost than male patients, however kidney transplantation reduced these differences. Future research should investigate whether systematic differences exist in access to care and possible strategies to mitigate excess mortality among female patients. BMJ 16 November 2021

Histopathology pattern and survival analysis of patients with kidney biopsy in the top end of Northern Australia from 2007 to 2020
Diabetic nephropathy is the most common native kidney biopsy in the NT with higher mortality among indigenous patients. This study supports the previous findings of indigenous female excess, younger age of kidney disease requiring kidney biopsy, and excess of diabetic nephropathy in the top-end of the NT. It can be speculated that some diabetic patients had atypical features prompting a biopsy.  BMC Nephrology 1 December 2022

Development and validation of algorithms to identify patients with chronic kidney disease and related chronic diseases across the Northern Territory, Australia
The authors developed and validated algorithms to identify CKD and related chronic diseases within electronic health records. Validation results showed that CKD algorithms have a high degree of diagnostic accuracy compared to traditional administrative codes. The highly accurate algorithms present new opportunities in early kidney disease detection, monitoring, and epidemiological research. BMC Nephrology 23 September 2022

The stay strong app as a self-management tool for first nations people with chronic kidney disease: a qualitative study
This study provides evidence of the suitability of the Stay Strong app for use as a chronic condition self-management tool. Participants set goals that addressed physical as well as social and emotional wellbeing needs, prioritising family, country, and cultural identity. The intervention aligns directly with self-management approaches that are holistic and prioritise individual empowerment. Implementation of self-management strategies into routine care remains a key challenge and further research is needed to establish drivers of success. BMC Nephrology 9 July 2022

Long-term outcomes for living kidney donors with early guideline-concordant follow-up care: A retrospective cohort study
Although policies directed toward improving early donor follow-up may encourage continued follow-up, additional strategies may be necessary to mitigate long-term donor risks. Canadian journal of kidney health and disease 1 March 2023

Development of a risk score for AKI onset in COVID-19 patients: COV-AKI score
AKI development in COVID 19 patients is accurately predicted by common clinical variables, allowing early interventions to attenuate the impact of AKI in these patients. BMC nephrology 2 March 2023

Association of omega 3 polyunsaturated fatty acids with incident chronic kidney disease: pooled analysis of 19 cohorts
Higher seafood derived n-3 PUFA levels were associated with lower risk of incident CKD, although this association was not found for plant derived n-3 PUFAs. These results support a favourable role for seafood derived n-3 PUFAs in preventing CKD. BMJ 19 January 2023
